If you are interested in a placement with the NHS, you must complete this form by Thursday 19 March. You must complete all fields. Please note that outcomes will be communicated via your College email address, so we suggest that you check this at least daily. Please note that if you are successful, you will be required to complete a series of documentation for the NHS.
